Afternoon fun at Mill Creek!

Mill Creek Nursing and Rehabilitation and its parent company, Foundations Health Solutions, follow the vision of nursing home developer Brian Colleran to create a "Culture of Care" in our nursing homes and communities. Whether in Galion or our communities across Ohio, we strive for excellence in long-term care, short-term care, hospice care, aquatic therapy, and a variety of other services.
